- Application
- The Application Form shall be completely filled out and signed by the payee.
- The payee shall pay Php 700 fee which can be increased at any time without prior notice.
- Acceptable modes of payment: Cash, Gcash, and Bank transfer.
- Payment shall only be made at the Reception/Cashier.
- Official Receipt shall be issued at all times.
- Payee shall receive the Health+ immediately after payment.
- NWD will not be liable for any loss, damage, injury, or claim arising from your failure to keep your personal data file accurate, complete, and updated.
- Validity
- One (1) year from the date of purchase of the card.
- Availment
- Health+ and its benefits are valid for use only if Health+ card is presented.
- Health+ is not a bank or credit card.
- Health+ will not earn points.
- Health+ cannot be used in conjunction with other promos, packages, and discounts (e.g Senior Citizen, PWD).
- Health+ is valid only to all NWD Branches.
- The card will be marked once the Annual Physical Exam Package is availed.
- Health+ card can be used upon issuance.
- Lost, Damaged, or Stolen Card
- Lost/damaged/stolen Health+ Card will not be replaced.
- Refund
- Health+ Card is non-refundable.
- Any unused benefit of the card is not convertible to cash or be used as payment for other services
